When I got my HIP HMO card and other paperwork (I think I have a half-shelf worth of stuff here), it said they couldn't assign the doctor I've been seeing as my primary physician. I called HIP, and explained the situation. They said to call the doctor.

I called the doctor's office, and was told that the person who could handle it was out yesterday, and to call back today after 10 a.m.

I've done that. My doctor's office still had no idea of why there'd been a problem, but called HIP and told them they are accepting patients.

I then called HIP again, and set this up with no problem. A new ID card is on its way. Tomorrow, when this coverage kicks in, I will refill two prescriptions.
